Please check the apache error_log for any error messages.
I have meet this error:
I found the user apache cannot open htpasswd.usres
That error seems to have stopped after changing the Options in the nagios.conf, but when the site loads it all looks like this.

I met this matter as you after I installed apache2.2 version ,but I resolved it
I uninstalled the old version of apache and php ,and intalled apache2.2.8 and php5.1.9.
