Single notifications and escalations

Here is my problem.
I have 1 person who needs to get notifications, these notifications need to go first to his email and second to a cell phone.

What I have done is setup 2 contacts, one with his email address and the other with the cell address.

With the first hard failure, I want only 1 notification to go to his email address…
5 minutes later if it has not been resolved, I want 1 notification to go to the cell phone contact

The only notifications that go out after this point should be when things have recovered, and I don’t care weather it goes to email or cell.

Can someone give me an example to accomplish this?
