Windows 2012 crashes on OpenStack + KVM + CEPH

I have an issue with windows 2012 virtual machines. Windows 2012 VM is crashing from time to time after restart, screenshot with error below. An issue is only with windows 2012. Windows 2012 R2, windows 2008 R2, linux VM’s are working fine

Infrastructure setup: KVM hypervisor managed by OpenStack and as storage we use CEPH.
OpenStack version: 2014.1.1
KVM hypervisor version: QEMU emulator version 1.7.1 (Debian 1.7.0+dfsg-8~bpo70+1)
Virtio drivers version:
CEPH version:
VM host: ProLiant DL360p Gen8 with Intel® Xeon® CPU E5-2630 v2 @ 2.60GHz

I have tried different KVM and Virtio driver versions. Windows fully updated and with no updates, recreated Windows VM from different images. Nothing helps.

Maybe someone of you are facing same issue or have any idea how solve it. All ideas are wellcome.
